Presenting Problem
Mr. King was accepted in to Safe Haven on 06/23/05 as a result of a need for shelter.
Referral Source and Process
Mr. King was referred to Safe Haven from UMHS and BSAP. His case managers
made the request as a result of his homelessness and his history of mental illness.
Living Situation
Mr. King currently resides at Safe Haven, located at 4101 Old York Road in Baltimore
City. Safe Haven is a homeless shelter that is owned and operated by University of
Maryland Health Systems.
History
ÐDevelopmental-Mr. King appeared alert and stable during the interview. His actual
IQ is not available.
ÐFamily Relationships/Dynamics-Mr. King presently has no immediate family invol-
vement at this time. His biological parents are deceased which began his issues of homelessness. His does admit to having a brother whose whereabouts are unknown. He does have an aunt that visits him occasionally. Her name is Ms. Bessie Rivers and her telephone number is 410-954-6827.
ÐLosses-Mr. Kings biological mother died in June of 1992 from a lengthy battle with breast cancer. He has fond memories of his mother and when asked how he coped with her death he responded, “It was hard and it still is.” His biological father is also
deceased. Mr. King could not remember the actual date of his death and appeared
less remorseful.
ÐSpirituality-Mr. King was baptized as a child under the Baptist faith. He stated that
he attends church occasionally.
ÐStrengths-Mr. Kings strengths include his ability to articulate his needs, he is
ambulatory, and with his vocational assistance he is employable.
ÐSexual-According to Mr. King his first sexual experience occurred at the age of 15.
He denies being sexual active at this time or abused.

ÐDomestic Violence-Mr. King denies any history of domestic violence.
Relevant Medical, Psychiatric, Educational and Social Service History
ÐEducational History-Mr. King was educated in Baltimore City Public
Schools. According to Mr. King last school he attended was Northern
High School. The highest grade he completed is the 10th grade.
ÐMedical History-Mr. King has MS and is prescribes Ibuprofen PRN for his joint pain. He has also been diagnosed with Hepatitis C.
ÐPsychiatric History-Mr. King has a history of mental illness. He has been
hospitalized twice for his mental illness (2003 & 2004). He has been
diagnosed with Depression and is also Bipolar. His current medications are;
Benztrophine 2mg 3 times daily and Trazadone 20 mg 2 times daily.
ÐAlcohol/Substance Abuse-Mr. King does have a history of alcohol and substance abuse. He denies allegations regarding his continue substance abuse issues. His substance abuse began at the age of 19 and his drug of choice is crack/cocaine. His alcohol abuse began at the age of 25.
ÐSocial Services-Mr. King does have a case manager at DSS and currently
receives TEMA and Medical Assistance.
ÐCriminal/Legal History-Mr. King admits to being arrested on two
occasions. Both of the offenses were drug related. At this time Mr. King
is not on probation or parole.
Assessment of Client Personality, Development, Social Roles
Presentation of Self- Mr. Kings presented himself in the interview
appropriately. He answered all of the interview questions to the best of his
ability. His demeanor was calm and friendly. He was very open in regards
to his life experiences and the effects of his mental illness in his present life.
He appeared to be somewhat embarrassed with his issues of being homeless.
Social Roles-Mr. King interacts socially with other residents at Safe Haven. He takes pride in completing the chores assigned to him. He often
volunteers to do other chores. He also socializes with members of his support group outside of Safe Haven.
Developmental Factors-Though Mr. King appears to be alert and oriented x3, there is evidence that he has poor insight and impaired cognitive functioning. There is no known evidence of childhood developmental difficulties and Mr. King denies receiving special education services. His current impairment may be secondary to his substance abuse, and he is in need of assistance when making certain decisions. Primarily these decisions involve financial planning, mental health treatment and housing. Historically, Mr. Kings decisions in these matters have been motivated by his substance abuse; specifically he will exhaust his resources in
