Panic Disorder
Definition of a Panic Attack (DSM-IV TR)
Period of intense fear in the absence of any real danger.
At least 4-13 of the following symptoms:
Palpitations
Sweating
Trembling or shaking
Shortness of breath
Choking sensations
Chest pain or discomfort
Nausea or abdominal discomfort
Dizziness or lightheadedness
Derealization or depersonalization
Fear of losing control or going crazy
Fear of dying
Numbness or tingling in the arms or legs
Chills or hot flashes
Rapid peak (usually within 10 minutes)
Limited symptom attack (< 4 symptoms)
3 types
Unexpected panic attack
Situationally bound panic attack
Situationally predisposed panic attack
Panic attacks can occur in different mental disorders (e.g. PTSD or
Social phobia).
Panic Disorder Overview
Essential features:
Presence of recurrent, unexpected panic attacks
Persistent concern for at least one month of having another panic attack or significant changes in behavior because of the panic attack.
Panic attacks cannot be caused by medical conditions or other mental disorders
Must have at least two unexpected panic attacks for diagnoses
2 types
Panic disorder without agoraphobia
Panic disorder with agoraphobia
Occurrence
10 percent of those seeking mental health counseling
Usually begins in twenties
1 in 63 have or will have PD
Twice as common in women
Causes of Panic Disorder
A. Research indicates both biological and psychological components.
Biological
Fight or flight
Low tolerance for body’s normal response to stress (false alarm)
May not be able to properly utilize the body’s own naturally produced anxiety reducing substances.
Psychological
Catastrophic thinking (heart skips = MI)
“What ifs”
B. Diathesis-Stress Model
Genetic predisposition
Having a first-degree relative with PD makes one 8 times more likely to also have PD.
Stress
Onset usually after stressful event (most often occurring after recent loss of or separation from a loved one).
