CONFIDENTIAL PSYCHOEDUCATIONAL EVALUATIONREASON FOR REFERRAL:AM was referred for an evaluation by her psychologist after she expressed concerns about her performance at school where she has recently enrolled for a Master’s level course. ASSESSMENT PROCEDURES:Wechsler Adult Intelligence scale – Fourth Edition (WAIS-IV)Woodcock-Johnson Tests of Achievement – Fourth Edition (WJ-IV Ach)Beery Development Test of Visual-Motor Integration (VMI)BEHAVIORAL OBSERVATIONS DURING TESTING:A.M. was tested over the course of one session that lasted for about 5 hours. A.M. acclimated easily to the testing environment, made good eye contact, and easily engaged in conversation. She established rapport easily, was pleasant and smiling throughout testing, and seemed at ease and comfortable. During the first half of the testing she was frequently yawning and putting her head on the desk, however, after the first break and having coffee she appeared more alert. No auditory, visual, or motor impairments were observed. A.M. is right handed.A.M. was frequently offered breaks during the testing. After the first break, when the tasks got hard she frequently said “You always start with simple tasks first and then make them harder.”. She got quieter as the tasks got harder, but appeared to put forth good effort. She would frequently fidget and move in her chair throughout the testing. Testing conditions were appropriate, and her assessment is considered an accurate reflection of her current functioning.ASSESSMENT RESULTS:Wechsler Adult Intelligence Scale – Fourth Edition (WAIS-IV)
The WAIS-IV is an individually administered intelligence test. The Full-Scale IQ and index scores have a mean of 100 and a standard deviation of 15.
