CHILD”S NAME: Susan Smith
AGE: 8
GRADE: Third
Susan is a very active student, always moving about constantly hopping around in her seat, tapping her feet or pencil or walking around the room. She seems incapable of staying in her seat. When working on group activities Susan often wanders away from her own group and disrupts other groups by talking about things that have nothing to do with the activity or subject that is taught. She does the same thing during class discussion, yelling out answers without raising her hand, and disrupting class by talking. Her desk is very disorganized with pages ripped out of notebooks, broken pencils and trash. It is often hard for her to find items like projects that she is working on. Several times she had lost or not completed her homework. When working on assignments in class Susan will have to be reminded several times to leave the other students alone and stay in her seat. Her lack of concentration often results in her not completing assignments. When she does complete an assignment she has often skipped steps or not paid attention to the details of the instructions. Even though Susan seems to be an outgoing child she has a difficult time making friends. She has a difficult time making eye contact, looking off to the side or at the ground when she is speaking or someone is speaking to her. Other students complain about Susan’s apparent lack of respect for personal space. She often stands in close proximity and talks too low to hear or extremely loud. She seems to be talking at people then with them. Susan also has a difficult time understanding the emotions and behaviors of others. For example there was an incident when Susan took a new book of color pencils off another student’s desk when they were in the bathroom. She did not try to hide the fact that she took the pencils. When the other child left the room, Susan simply walked to their desk and took the color pencils. The child was upset when she came back to the room and saw her pencils gone. She got mad at Susan and accused her of stealing them. Susan did not understand why the other child was upset with her or that she should have asked to use them before taking them. It has also been observed that Susan sits by herself at lunch and plays alone at recess. Much of the same behavior has been reported at home where Susan often plays by herself and takes thing from her siblings without asking or when they are using them. Her mother reports that Susan’s room is always a mess with half drawn pictures, uncompleted puzzles, and discarded toys littering the floor. Susan’s chores at home are either left undone or half done, such as when taking out the trash she will leave the tired up trash bag on the kitchen floor instead of taking it out to the garbage can.
