Thanks for applying to MADE! We really want to hear your story, but cant consider you for the show unless we can see you on tape. The idea is just to get a natural sense of your personality and who you are. Please make the tape! Just talk to the camera, be relaxed and try to tell us about yourself and why you need our help to be MADE. Remember- be relaxed and authentic..

Please do not make tape longer than 5 minutes. Answer the questions that are listed below on the application.
Please fill out your application/questionnaire legibly!!!
Please use DARK colored ink so we can read it.
THIS IS NOT A TEST!! There are no right or wrong answers. We just want to get to know you better, so relax and take your time answering the questions. And please be honest!
If you are under 18 please have your parent/legal guardian agree to read, sign and date your questionnaire to indicate they agree to you applying.
Remember to include a recent photograph of yourself 3×5 or larger.
Please make all tapes on VHS/or DV. We do not have the capacity to look at hi- 8 tapes.
Please double check your tape submission to make sure the tape works and has good sound and picture.

Again: Dont make the tape any longer than 5 minutes- and DONT have anyone read the questions on the tape. Just answer them yourself and be natural. Dont forget to fill out the following application and send it in with your tape submission.

2) What do you want us/MTV to help you with? What can we provide for you? Have you tried to do this before? When?
3) What is preventing you from achieving your goal on your own? Why do you need our help?

YOU MUST APPEAR TO BE BETWEEN 15 and 21 YEARS OLD TO AUDITION FOR MADE!!!!! Please bring a photo of yourself along with this questionnaire.
MADE is all about making dreams come true. We want to prove that with hard work, and a little help from MTV, you can achieve your goals.
We are looking for a specific, individual goal or aspiration you may have that is attainable in the next 6 months!!!!! We are not looking for a vague goal, like “I want to be a star” or “I want to be just like Britney Spears”. We are also not looking for VJs, Models or people trying to be “famous”.
Your goal should be something that is within your reach within the next 6 months, but you need some training in order to make it a reality.
MTV will provide one or two months of training with top experts in the field to give you the skills to make your dream come true. If your goal takes years and years of training, we cant help you out this time, sorry!
