A BRIEF HISTORY OF THE ISM CODE
Throughout the late 1980s into the early 1990s there was a series of major high profile
maritime accidents. The shipping industry realized that they were operating in an unsafe
environment, prone to accidents and ship related pollution and in an effort to improve the
environment, the stakeholders focused on improving the hardware of the vessels e.g. the
construction of the ships and their hardware. However, after much investigation it was
concluded by researchers that the common factor in the majority of maritime accidents
and ship related pollution was human error or the human element. The human element
affects areas such as maritime safety, security and marine environmental protection
which involves the entire spectrum of human activities performed by ships crews, shore
based management, regulatory bodies and others.
The “Human Element in Shipping Causalities” report states that the human element was
found to be causative in over 90 percent of collisions and groundings and over 75 percent
of contacts and fire/explosions. In an attempt to address the issue of the human factor
being the dominant contributor to these accidents regional, national and even individual
