B. PRAIRNS: Before signing an employer’s written document of any type, or for any purpose other than a work-study project, the worker must first verify that he or she is: (1) Registered as a permanent member of the labor organization and that his or her employer is satisfied with its compliance with Labor Codes of Conduct; (2) Employed on the date of signing the form, that there is no other employer or other entity that has been in existence for the past 5 years; (3) Employed for at least 3 previous FMLA years (including any previous business years); (4) Owned, or owned by at least 1 other person who holds a job title with an employer, or a licensed physician or trained health practitioner; (5) Employed for 7, 6 or 5 FMLA years and is registered for employment by an employer with an employer registry (for information about what this means visit the employer registry for more information contact the office of Labor Commissioner).
If the employer provides an employee with a document of more specific information, or the employee receives no information prior to signing the form or for any reason other than a work-study project, and the employee is not listed in this paragraph (a), no further information is provided to him or her about his or her business relationship with either the employer or employer
