Treatment PlanBelindaUOPIntervention, Direct Service Delivering and case managementBSHS/405Bernice VriesMay 5, 2015Treatment PlanWhen a person is seeking services for a problem, the intake interview is generally when the treatment alliance begins between the client and the human service professional. It has been proven that a clients treatment results are better when the helper attends to the relationship between themselves and the client (Lee CW & Cuijpers P 2013).The CaseI have an individual by the name of Mr. Luna he is 34 years of age and is an army veteran. Mr. Luna came to our agency seeking help for symptoms that have developed due to war; he did two tours to Afghanistan between 2005 and 2008. When Mr. Luna was in Afghanistan he said he had to kill many people who were casualties of war and also saw many of his own fellow soldiers die. Mr. Luna struggles with distressing thoughts and memories, nightmares, building relationships, irritability, avoid large crowds and sadness and guilt.

During the assessment interview Mr. Luna seemed nervous and fidgety, he explained that he was reluctant in seeking services because he is afraid that there is no solution to his problems. Mr. Luna expressed that he is not able to sleep well at night due to the night terrors, therefore has not been able to hold a steady job because he is always tired and irritable. He tells me that he feels like a horrible person because he had to kill people, but that he had no other choice. Mr. Luna was emotional as he spoke of all the tragedy he had endured, while in war. Mr. Luna feels guilty because many of the soldiers he knew died and he doesn’t understand why he survived, he feels as though he should have died with all the other soldiers. Mr. Luna was engaged before his last tour to Afghanistan but said that his relationship fell apart when he returned; he lives with his parents because he doesn’t want to be alone, his desire is to change his life circumstances. Mr. Luna has some short-term goals he would like to accomplish with the help and guidance of our agency. He wants to be evaluated and know if he has PTSD, so that he can work towards dealing with his problems, he is willing to commit himself to treatment. Mr. Luna would like to be able to cope with his irritability and sleeping problems so that he can get a stable job and hold on to it. He wants to hopefully live on his own and know that he is going to be ok.
